Coordinated Assessment and Housing Placement (CAHP) System

Our organization participates in the Colorado Balance of State Continuum of Care (BoS CoC) Coordinated Entry System (CES). CES is "...a process that ensures that all people experiencing a housing crisis in a defined geographic area have fair and equal access, and are quickly identified, assessed for, referred, and connected to housing and homeless assistance based on their needs and strengths, no matter where or when they present for services..." 
Community Compassion Outreach is pleased to be one of the many participants in the Coordinated Assessment and Housing Placement (CAHP) program which is managed by ​Housing Solutions for the SW in La Plata County.

  • SAFE TALKS IN RECOVERY (STiR) is a voluntary group meeting held during Compassion Center hours.
  • COMPASSION CENTER -- Doors open at 9:00 a.m. until 4:00 p.m. for light breakfast, snacks, beverages and lunch. Client intakes by Appointment Only. Donations room available. 
  • VAN pickup from Manna at 10:10 a.m. -- 10:25 a.m.
  • COFFEE & CONVERSATIONS are held Saturdays: Doors open at 11:00 a.m. -- LUNCH will be served at NOON until 1:30 p.m.
